POSITION SUMMARY

Primary function is to administer skilled nursing care, under the supervision of a registered nurse, for patients of all ages in their places of residence. Participates in the coordination of care with the interdisciplinary team, patient, family, and referring agency.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Working as a PRN (per Visit) contractor, Under the direction of the RN, assists in identifying the patient's physical, psychosocial, and environmental needs as evidenced by documentation.
• Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
• Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
• Regularly reevaluates the patient's nursing needs and evaluates the outcomes of care. Monitors assigned cases to ensure compliance with requirements of third-party payor.

QUALIFICATIONS

Educational Requirements and Experience:

• Active Vocational/Practical Nurse license to practice nursing in the state of Texas.
• One year experience as a vocational/practical nurse in a clinical care setting preferred, hospice or home health preferred.
• Graduate from an accredited School of Nursing.
• Current CPR certification.
